How select 'BlankPage' on 'Create form template' Macro ?

I couldn't English very well...


I using 'Create form template' macro.

but, 'Template name' property have not 'BlankPage' item.

How use 'BlankPage' in 'Create form template' macro?

You can make a template (empty) that is called 'Blank Page'... but what is the point?
