How do I edit the "Create" window when making a template?

I have created a template and used the "Create from template" macro to put it as a button on a page. After clicking this button I would like another screen to populate for users to add labels and the page title, identical to the way the "How-to" blueprint and "Troubleshooting" blueprints do. However, I cannot seem to get this window to populate on my own. Is there a step I am missing?

See documentation link below. Make sure your template has the labels in place to make it standard on all new pages.

Thank you for the reply Victor, unfortunately this article doesn't show the piece I am attempting to do. I can't get the "Create" window to populate after the button is placed. I want users to be able to enter the title and labels  at the time of article creation because I am using a macro to pull from these labels onto other source pages. I hope  that makes sense.
