How can I fix a TOC problem in Confluence?

I have a TOC set up for heading 1 that takes from the headings in the page. It worked when I created the page and then has worked only intermittently, in other words some H1s in the page simply do not show up in the TOC. Does anyone have any ideas how to fix this? Thanks!

Only <h1>....</h1> and nothing else, no <div....> or anything?

I sometimes find text that looks like a headline (same color and size), but looking at the source code it turns out to be some <div style="font-size: ..." ...>

Other thing to check is "Exclude Headings" in the TOC menu.
