How can I create a link that acts like the "Create from Template" button?

• When clicking Create > Decision, Confluence pops up a form.
  Good.
  
  
• When using a "Create from Template" button for a new Decision, Confluence pops up a form.
  Good.
  
  
• When copying the link used for the button and using it on a page as a link, a new page in edit mode based on the template is presented.
  Not good.

I'm trying to put a New Decision link on my home page that will add the page as a child of a page that is not the home page. Adding &fromPageId={pageID} works great for this functionality. Now I just need it to pop up a form instead of a new page. How?

I am also interested in this as I would like to use an image as the button. I have tried using the URL that the button uses and embedding that into the image as a link but it just takes me to the dashboard.
