Heading in Word imported via Macro appear in TOC but not hyperlinked

When I add a word doc via the Office Macro to my page, the headings appear in the page's TOC. However, these headings are not hyperlinked like those created within the page making navigation difficult. There does not seem to be an easy way of ensuring all the headings are hyperlinked. Any thoughts?

The way I handled this recently was replacing the TOC from the Word doc and replaced it with the Table of Contents (TOC) macro that comes installed in Confluence. It will pull from the headings and have jumplinks to them.
