Creating Forms

Hello.

I am currently working on editing a Wiki page and need some help.

How do I create forms that send the result to an email of my choice? I have tried using the form-mail macro, however, I don't know how to edit/add more fields that the form asks for.

This is what the form currently looks like but I would like to add more fields and get rid of the "message" field.

 

Any help would be appreciated!

It'd be good to read it, setting up these forms is straightforward but it's not the easiest thing to do.

If you want a bit more comprehensive solution then try ConfiForms, quite easy to setup and very versatile in functionality. 

For example you can create feedback forms to send you emails when certain fields are selected / filled.
