Connecting Confluence to JIRA for User authentication

This question is in reference to Atlassian Documentation: Connecting to Crowd or another JIRA application for user management

I am having trouble configuring this...  I've entered the site URL of my JIRA instance, but the Save & Test button is greyed out and the test button returns an error:

Connection test failed. Response from the server:
com.atlassian.crowd.exception.InvalidAuthenticationException: Application failed to authenticate

I've also tried from the JIRA side (even though the documentation specifies that you should start with the client).

What have I missed?

Ok - seems the documentation is wrong.  You need to set up the server first (i.e. JIRA), not the connecting client (Confluence in my case).

I have the exact same problem where the Save and Test button is disabled, but everything is set up correctly in JIRA. I'm certain it's correct in JIRA because it worked before we upgraded Confluence and nothing has changed in JIRA.
