Confluence 5.x: How to create a Request to Update a Page form or macro

Hello,

I'd like to add a macro or a form to all my public facing pages that viewers can use to suggest changes to a page. I'd like it to automatically include the title of the page the viewer is refering to. Does something like that already exist?
