Does the red repository icon mean something?

I started noticing this in the past few months.

See attached screen grab - the icon for the "transplant" repository is red, with some kind of spiral thing inside it - why?

The logo can change automatically depending on the language you select. In your case, the language for the transplant repo appears to be set to Scala. You can change it by clicking the Cog icon.

That just looks like the repo logo was changed. Go to Repository, select the cog menu and you will see the change logo option (far right hand side) under Repository Details.
