Title says it all, on both new and old repositories, in a team or not, these pages no longer work. The text field appears, but when clicking "Add" nothing gets added.

There was a bug in our Javascript code that was preventing this from working, but the team resolved it. It sounds like you were trying to use it around the time when the bug was introduced. Sorry about that, but I've confirmed it has now been fixed.
