Is there a plugin to enable addition of multiple RAG Indicators to a form?

There are several plugins that can help you do RAG - here are a couple of examples

https://marketplace.atlassian.com/plugins/com.almworks.jira.colors.colors-plugin/server/overview
All of the above require you to determine what constitues Red Amber or Green through the use of JQL.
If you are not calculating the RAG status but simply want to have a field that holds that value then you can of course create the custom field to store the information. Then using one of the above plugins colour your ticket accordingly.

Since my original response in 2016 there are now a number of Apps released for Cloud that can cover the initial Use Case. Rather than right another answer that would be quickly outdated here is a link to marketplace for

With all Apps remember to test fully in your development environment before deploying to production.
For those of you using Cloud I would recommend running a parallel test environment to your main instance for this testing.
