I want to have several yes/no select lists asking if the user needs to add an additional option and if yes it will display a different select list with choices. If no it will stay the same and keep the fields hidden. Or possibly just have some kind of button that + says add another application which would display another field for them to choose from.

Hi,

you need just add dynamic select with options yes/no and add related fields in configuration. Can you follow this instruction: https://intenso.atlassian.net/wiki/display/DF/Dynamic+Custom+Fields

I was able to do this, but it does not work on the customer portal side. How can I fix this?

Dynamic forms by InTENSO works only in JIRA. There is different plugin for customer portal also written by InTENSO, which contains many great features for JIRA Service Desk. It calls Extensions for JSD. There is also simple dynamic forms for customer portal (single select) and multilevel structure (checkboxes or radio buttons):

I hope I've helped you.

Oh, I see. Do I have to reconfigured the fields I created with Dynamic Forms or does this plugin extension automatically make the existing Dynamic Form fields compatible?

If you have both plugins there will be option in extensions to import configuration from dynamic forms plugin. There is information about it: https://intenso.atlassian.net/wiki/display/SPFJSD/Dynamic+Forms

Best Regards
