adaptavist test manager: cannot add Macro in Confluence

We installed TM4J on both JIRA and Confluence.
We created the Application link on both and its shows OK/Green on both servers.

When we go into a Confluence page and add a TM4J macro, we get a BLANK/Empty popup, ???
